Privacy Policy

Last Updated: October 29, 2025

Applies to: Prestige Dental Care (“we,” “us,” or “our”) and our websites at [insert domain] and any linked pages we own (collectively, the “Site”).

Important HIPAA note: This Website Privacy Policy explains how we handle personal information collected via the Site (e.g., contact forms, online scheduling, payments). It is not the required HIPAA Notice of Privacy Practices (NPP) that describes our privacy practices for protected health information (PHI) in clinical care. For our NPP, please see: HIPAA NPP PDF

1) Information We Collect

We collect the following categories of information:

  • Identifiers & contact details (e.g., name, email, phone, address) when you submit forms, request appointments, or subscribe to communications.
  • Health or appointment context that you voluntarily provide in website forms/messages (e.g., reason for visit). If your submission contains PHI, we handle it under HIPAA when received by our systems and HIPAA‑compliant vendors.
  • Internet/technical data (e.g., IP, device/browser, pages viewed, time on page) via cookies, pixels, and similar tools.
  • Payment/financing info when you pay online or apply for financing through third parties (processed by those providers, not stored on our Site).
  • Testimonials/reviews you submit or authorize us to share.
  • Geolocation (approximate) from your IP or if you allow your browser to share it.
  • Children’s data: Our Site is not directed to children under 13 and we do not knowingly collect their personal information without verifiable parental consent.
2) How We Use Information
  • Provide services: respond to inquiries, schedule visits, process payments, send appointment reminders, and manage your patient experience.
  • Operate and improve the Site: security, debugging, analytics, and personalization.
  • Communications: send transactional notices and, with required consent where applicable, marketing communications (email/SMS).
  • Legal, safety, & compliance: detect/prevent fraud or security incidents, comply with law, and exercise legal claims.
3) Cookies, Analytics & Advertising

We use cookies and similar technologies to run the Site and understand usage. We configure tagging to avoid PHI and instruct vendors accordingly. Your browser may support Global Privacy Control (GPC) signals; where required by state law (such as California’s CPRA) we treat GPC as a valid opt‑out signal for “sale”/“sharing” of personal information. You can manage cookies in your browser settings. Some features may not work without cookies.

4) reCAPTCHA & Bot Protection

We may use Google reCAPTCHA to protect forms from spam/abuse. If reCAPTCHA is present, Google may collect device and usage data subject to Google’s Privacy Policy and Terms. Required disclosure: “This site is protected by reCAPTCHA and the Google Privacy Policy and Terms of Service apply.”

5) Vendors, Business Associates & Disclosures

We share personal information with:

  • Service providers (hosting, email/SMS delivery, web forms/scheduling, payment processors, analytics, security) under contracts restricting use to our purposes.
  • HIPAA Business Associates for PHI (e.g., patient forms/scheduling platforms), with BAAs in place and safeguards aligned to the HIPAA Security Rule.
  • Financing/third‑party platforms you choose to interact with (they process your data under their own policies).
  • Legal and safety recipients (law enforcement, regulators) when required.
  • Corporate transfers (e.g., mergers/acquisitions) subject to applicable law.

We do not sell PHI. For state law definitions of “sell”/“share” of non‑PHI personal information, see “State‑Specific Disclosures & Rights.”

6) Data Security

We implement administrative, physical, and technical safeguards appropriate to the sensitivity of information, including measures designed to protect ePHI consistent with the HIPAA Security Rule (risk management, access control, transmission security, etc.). Encryption is an addressable safeguard; we use encryption in transit and implement reasonable protections appropriate to risk.

7) Retention

We retain information as needed to provide services, meet legal/recordkeeping requirements, resolve disputes, and enforce agreements. (Clinical record retention follows healthcare law/board rules; web logs/marketing data follow shorter operational schedules.)

8) Your Choices & Rights

Email marketing: You can unsubscribe via the link in our emails (we still send transactional/administrative messages).
Text messages: For marketing or non‑essential SMS, we obtain prior express written consent and include clear opt‑out instructions (e.g., reply STOP).
Cookies/analytics/ads: Use your browser controls and (where available) our cookie banner. We honor GPC as legally required.
State privacy rights: If you are a resident of a state with a comprehensive privacy law (e.g., California, Virginia, Colorado, Connecticut, Utah, Texas, Delaware), you may have rights to access, correct, delete, and/or opt out of certain processing (e.g., sale/sharing/targeted ads/profiling). To exercise rights or submit a privacy request, contact us at [email protected] or [webform URL]. We may verify your identity and, where allowed, act through your authorized agent.

9) Children’s Privacy

The Site is not intended for children under 13. We do not knowingly collect personal information from children under 13 without verifiable parental consent. If you believe a child provided personal information, contact us and we will delete it consistent with COPPA.

10) Breach Notification (PHI)

If a breach of unsecured PHI occurs, we will notify affected individuals and authorities as required by the HIPAA Breach Notification Rule and applicable state laws.

11) Third‑Party Links & Embedded Services

Our Site may link to third‑party sites (e.g., review platforms, financing, maps, scheduling portals). Their privacy practices govern information collected by them. When we embed or link to third‑party tools (e.g., Google Maps, reCAPTCHA, video players), those providers may collect data per their own policies.

12) Do Not Use the Site for Emergencies

Online forms/messages are not monitored 24/7 and not for emergencies. Call 911 or seek immediate care in an emergency.

13) Changes to This Policy

We may update this Policy from time to time. The “Last Updated” date reflects the effective date. Material changes will be highlighted on the Site and/or sent via email when required by law.
